APPLICATION PROCESS
We would like to thank you for allowing us the opportunity to partner with you to bring continued care and structure to clients.
The following information is a detailed account of the intake process for a potential resident at Rise Up Transitional Resources :

The completed application, any applicable assessments (completed with case manager), and the following documentation must be submitted to
● Photo ID
● Social Security Card
● Proof of Income
● Medication List
● Discharge Paperwork
● Medical Insurance Information
There is a $200.00 non-refundable deposit due at move in.
Once documentation is received, an interview is required between the potential resident and a representative of Rise Up.
Please allow for up to 72 Business hours for an application to be processed.
The application process entails verification of references, landlord verification, verification of income, background screening and review of all documentation that has been submitted.
If an application is denied, a rejection letter will be sent to the applicant and case worker who is assisting in the process.
Once an application is approved, including an assigned room and move in date, a behavioral contract will need to be signed by the approved resident.
